Patient Rights
As a patient, you have the right to:
- Quality health care.
- Respectful care provided by competent personnel.
- Identification of all health professionals participating in your care.
- Confidentiality regarding your medical care and related records, except as provided by law or third-party contractual agreements.
- Access to all information contained in your medical record, unless access is specially restricted by the primary care physician for medical reasons.
- Information about your illness and course of treatment, including alternative treatments and possible complications.
- Examine and receive a detailed explanation of your bill.
- Receive health care services without discrimination based upon race, color, age, ethnicity, ancestry, religion, gender, sexual orientation, national origin, or marital, familial, veteran or disablity status.
Patient Responsibilities
As a patient you have the following responsbilibites:
- Provide all necessary personal and medical information necessary for your treatment.
- Ask questions if you do not understand your illness or treatment options.
- Be considerate of other patients and staff members.
- Tell your physician if you are not willing or able to follow recommended treatment options.
- Actively participate in your care by following instructions and keeping follow-up appointments.
- Assume the financial responsibility of paying for all services rendered, either through an insurance company or by being personally responsible for payment for any services that are not covered by insurance.
